This is a well made item and interesting to use with added sensations but the top strap can slip fairly easily to the side and come off, even with adjustment and without intent. It can be worn over a posture collar. It would be better if there were some kind of additional strap to hold the overhead strap in the middle of the head. Even though it is well-constructed, it is not made of real leather, which is a little disappointing considering the "Strict Leather" logo and reputation.
